MANAGEMENT SERVICES HEREINAFTER REFERRED TO AS OWNER, FOR THE PROJECT REFERENCED BELOW:

PROJECT NAME & LOCATION: Replacement of Heating, Ventilation and Air Conditioning (HVAC) Systems, State Fire
College, Ocala, Florida
ESTIMATED CONSTRUCTION COST: $378,000.00
MANDATORY DEPARTMENT OF MANAGEMENT (DMS) SERVICES PREQUALIFICATION: Each bidder whose field is
governed by Chapter 399, 455, 489, and 633 of the Florida Statutes for licensure or certification must submit pre-qualification
data of their eligibility to submit bid proposals five (5) calendar days prior to the bid opening date. If bidder has been previously
prequalified by the Department of Management Services for the current biennium (September 1 through August 31) of even
numbered years, please verify pre-qualification is still valid. Prequalification requirements are outlined in the Non-Technical
Specification Instruction to Bidders under Article B-2 "Bidder Qualification Requirements and Procedures". Please call (850)
488- 6233 for information on requirements for pre-qualification with the Department of Management Services.
After the bid opening, the low bidder may be required to provide additional financial and bidding qualifications in accordance
with Florida Administrative Rule 60D-5.004. These requirements are outlined in the Non-Technical Specifications Instruction to
Bidders under Article B-22 "Qualifications for Award of Contract".
PUBLIC ENTITY CRIME INFORMATION STATEMENT: Under section 287.017 Florida Statutes, a person or affiliate who has
been placed on the convicted vendor list following a conviction for a public entity crime may not submit a bid on a contract to
provide any goods or services and/or construction or repair of a public building or public work and may not be awarded or
perform work as a contractor, supplier, subcontractor, or consultant under a contract with any public entity for a period of 36
months from the date of being placed on the convicted vendor list.
